Northamptonshire Palliative Care and End of Life Service
Information for Patients and Families
DHU Healthcare provides the Northamptonshire Palliative Care and End of Life Service in partnership with NHS services and local health and care organisations. The service supports patients who have been referred via NHS 111, 999, or another healthcare professional, with the aim of helping people remain as comfortable as possible in their own home where this is their wish.
We work closely with GPs, community nursing teams, hospices, ambulance services, care homes, social care teams, and Integrated Care Systems (ICSs) to ensure coordinated, person-centred care and to support safe alternatives to hospital admission where appropriate.
We offer both clinical and personal care support, including:
Symptom management and clinical assessment
Catheter care and mouth care
Help with moving and handling
Medication support, including syringe driver set-up and monitoring
Regular review and updating of care wishes as needs change
As someone approaches end of life, their condition may change. We support families and carers to understand what is normal, what to expect, and when to seek further help. Our aim is to provide reassurance, comfort, and timely support.
We aim to contact patients within one hour of referral. If a visit cannot be completed immediately, we will ensure appropriate interim support or signpost to other services.
To provide compassionate, responsive care that keeps patients comfortable, supported, and cared for in their preferred place, while working closely with all relevant health and care partners.
