GP Federations are groups of General Practices that have formed an organisational entity and are working collaboratively with their local health economy. 4FED covers c360,000 patient population & Across Leicester Health covers c290,000 patient population

Our Services

Clinical Navigation Hub:

Operation of the Clinical Navigation Hub a “hybrid” model, integrated across DHU NHS111 Services (regionally) and the local LLR Clinical Navigation Hub. The Hub provides 24/7 telephone based advice and onward navigation including direct booking of face to face appointments where appropriate, and is delivered by a mixed clinical workforce

Home Visiting Services:

Provision of a 24/7 Home Visiting Services across LLR comprising: In hours Urgent Visiting Service; more traditional Out of Hours Home Visiting Service; Urgent Night Nursing Service. All services as a key aim, are focused on the reduction of avoidable emergency admissions and attendances to acute settings of care.

West Leicestershire Integrated Urgent Care – in partnership with 4FED.(Loughborough Urgent Care Centre & Hinckley and Coalville based Primary Care Clinics):

Delivery of a model of local integrated urgent care that enables 24/7 rapid assessment, diagnosis and treatment of patients with urgent care needs.

Leicester Royal Infirmary – Front Door & Primary Care Services – in partnership with University Hospitals of Leicester & Across Leicester Health:

24/7 Provision of an assessment service for adult ambulant patients and a Primary Care service for adults and children that have a minor illness or injury. A key aim is to deliver excellent and sustainable clinical outcomes to patients including safe and effective treatment for patients with a variety of non-life threatening health conditions, minor injuries or illnesses.

East Leicestershire Out of hours Services:

Operation of out of hours urgent care services based at 2 East Leicester locations (Oakham & Lutterworth)

Leicester City GP Practices – in partnership with Across Leicester Health:

Operation of 3 GP Surgeries within Leicester City CCG Area, providing high quality, convenient and responsive care to a diverse population

Leicester City Healthcare Hubs – in partnership with Across Leicester Health:

Provision of integrated urgent care and extended primary care access services at 4 Leicester City locations

Portfolio career opportunities are a reality, due to the multiplicity of contracted services, spanning urgent and primary care, in a number of settings and locations.”
